Health Forward Awards $422,314 in Applicant Defined Grants

KANSAS CITY, MO – The Health Forward Foundation (Health Forward) has awarded 10 nonprofit organizations a total of $422,314 in Applicant Defined Grants.

The following grants were awarded in May 2015:

Children’s Center Campus $48,750 To allow for the safe and most effective use of the therapeutic pool.
Curators of the University of Missouri – UMKC $43,564 To enable Sojourner Clinic to continue to provide quality care to a medically indigent population.
Hope Family Care Center LLC $45,000 To support clinical staffing for primary and urgent-care after-hour clinics.
Infant Toddler Services of Johnson County Inc $20,000 To provide free, easily accessible developmental screenings to at-risk children, aged birth through 3 years old.
Jackson County CASA $30,000 To recruit, train, and support volunteer court advocates in Jackson, Johnson and Wyandotte counties for children ages 0 to 5 in the system because of abuse or neglect.
Jacobs Well of Kansas City Ministries $30,000 To enable Carousel Adult Day Care to provide physical and occupational therapy to adults with severe disabilities.
Johnson County Mental Health $50,000 To provide treatment for adolescents, transition-age youth and adults through age 27 with co-occurring mental health and substance use disorders in Johnson County, Kan.
Redemptorist Social Services Center Inc $60,000 To provide multiple types of medical assistance benefiting the medically fragile: the elderly, the underserved, the medically indigent.
Visiting Nurse Association (VNA Corporation) $45,000 To provide high-risk home healthcare patients a consultation visit with a licensed pharmacist or senior pharmacy student under the pharmacist’s supervision and guidance, to provide medication therapy management services. The goal of this program is to reduce re-hospitalizations, and therefore healthcare costs, as well as improve patients’ health post-hospital discharge and their satisfaction with their home healthcare experience.
West Central Missouri Community Action Agency $50,000 To support a family health nurse practitioner to provide family health to the low-income and uninsured.

Health Forward Applicant Defined Grants (ADG) are awarded outside of the Foundation’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2015, Health Forward will award a total of $4.1 million in ADGs.
