Kansas City, MO —The Health Forward Foundation (Health Forward) has awarded eight nonprofit organizations a total of $371,000 in applicant defined grants (ADG) during the month of August.
Health Forward reviewed 17 proposals totaling $786,173 for this funding round.
The following grants were awarded in August 2017:
|Baptist-Trinity Lutheran Legacy Foundation||$50,000||To facilitate access to hearing aids for low-income individuals.|
|Covenant Presbyterian Church||$62,000||To support community health workers who will provide community-based health and wellness services, and chronic-care management to uninsured and underinsured persons. Funds will also support teledental services.|
|Food Equality Initiative||$35,000||To provide access to allergen-friendly and gluten-free foods for low-income individuals diagnosed with food allergies or celiac disease.|
|Jackson County CASA||$52,269||To extend health navigation services to older foster youth, who are aging out of the foster care system.|
|Johnson County Department of Health and Environment||$50,000||To provide evidence-based education to help reduce teen pregnancies, sexually transmitted infections, and associated risk behavior.|
|Northwest Communities Development Corporation||$15,750||To develop a strategic plan that will better serve residents in Sugar Creek, Missouri, and the northwestern part of Independence, Missouri.|
|Planned Parenthood||$60,000||To expand access to comprehensive, sex-positive, and medically accurate sexual and reproductive health education in Wyandotte County.|
|The University of Kansas Center for Research||$45,981||To support the Johnson County Mental Health Center in fully using the Strengths Model for Youth, a strengths-based case management model for youth with serious mental, emotional, and behavioral disorders.|
Health Forward applicant defined grants are awarded outside of the Foundation’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2017, Health Forward will award approximately $4.085 million in ADGs.
