Reporting Forms Webinar

With the introduction of the Healthy Communities theory of change, we have changed our reporting forms and introduced a list of outcomes and indicators for all of our grant rounds.

This is a shift in how Health Forward has collected information in the past and you may have questions on how to complete these forms for your project.

We will be conducting a live, one-hour webinar at 11 a.m., Thursday, Jan. 18. In this call, we will walk through the forms, provide examples, and answer questions.

Please review the Strategies, Outcomes, and Indicators Workbook and bring at least one question. This is an interactive webinar for information sharing and technical assistance.

This webinar meeting is not mandatory and will be posted on our website following the conclusion of the webinar.
