Health Forward awards $269,255 in applicant defined grants

Kansas City, MO — Health Forward Foundation has awarded six nonprofit organizations a total of $269,255 in applicant defined grants during September 2019.

We are proud to support the following:

Agency Award Grant Description
Community Health Council of Wyandotte County $70,000 To support the Cradle Kansas City initiative that connects diverse stakeholders with strategies to reduce preterm and sleep-related infant deaths.
Cultivate KC $60,755 To sustain the Metro Farms and Food Systems program for individuals at every stage of local food system: farmers, consumers, community groups, and leaders.
Hillcrest Transitional Housing $30,000 To increase access to health care services for homeless clients by establishing stronger and more consistent policies, procedures, and programs.
Unified School District 258-Humboldt $43,500 To support a school nurse position that will manage student vaccination data, provide health screenings, and assist students with individualized education programs.
University of Missouri Extension Council of Jackson County $25,000 To educate, engage, and mobilize resident volunteers to effectively address illegal trash dumping and improve the health of eight neighborhoods through a partnership with Historic East Neighborhoods Coalition.
Village Initiative $40,000 To provide core operating support and sustain wrap-around re-entry services for returning citizens and their families in Wyandotte County, Kansas.

Health Forward reviewed 10 proposals totaling $537,772 for this funding round.

Applicant defined grants are reviewed and awarded monthly. These grants allow organizations to define their own immediate grant needs in relation to Health Forward’s mission, target population, and service area. Organizations are allowed to apply once per year and are capped at $75,000. In 2019, Health Forward will award approximately $4 million in applicant defined grants.
