Health Forward Approves Nearly $900,000 in Applicant Defined Grants

KANSAS CITY, MO – The Health Forward Foundation (Health Forward) approved $896,077 in applicant defined grants to support organizations who strive to eliminate barriers to quality health and promote healthy lifestyles.

Health Forward awarded the following grants:

$65,000 to Catholic Charities of Kansas City-St. Joseph to provide health screenings and follow-ups, health literacy, health monitoring and case management.
$60,000 to Developing Potential to enable low-incomes adults with developmental disabilities to receive nursing support.
$74,920 to the Health Care Coalition of Lafayette County for information technology training and support of a 24-hour nurse line.
$40,000 to Intensive Family Counseling to increase program and technical capacities.
$60,000 to Mattie Rhodes Center to support the Latino Mental Health Support and Outreach project.
$65,000 to MetroCare to support recruiting primary care and specialty physicians through the Northland CARE, MetroCARE and Wy/Jo Care projects.
$40,000 to the National Alliance for the Mentally Ill of Greater Kansas City for a Detention Officer Crisis Intervention Teams (DOCIT) Initiative.
$73,460 to reStart for its youth mental health services program.
$59,400 to YWCA of Greater Kansas City for capacity building.
$44,000 to the Unified School District 258-Humboldt for its Healthy Humboldt program.
$15,000 to Citizens for Missouri’s Children to host Making KIDS COUNT community forums in Lafayette, Cass and Jackson County, Missouri.
$49,995 to El Centro to educate Latino families on home smoking restrictions and smoking cessation.
$40,000 to the Missouri Citizen Education Fund for health care reform advocacy in the Kansas City area.
$50,000 to the Missouri Health Advocacy Alliance for health-related advocacy.
$50,000 to Metropolitan Lutheran Ministry to provide access to photo IDs and copies of birth certificates for poor and homeless persons in order to obtain social and medical services.
$45,000 to Midwest Foster Care and Adoption Association for its Healthy Lives through Learning project.
$20,000 to N2N for a part-time social worker to engage with homeless and near-homeless persons in the Neighbor2Neighbor community.
$44,302 to SIDS Resources to provide free SIDS awareness and education programming to low-income in Jackson and Wyandotte counties.
In 2009, Health Forward will fund a total of $4 million in Applicant Defined Grants (ADG). Proposals are accepted and reviewed monthly.
