Healthy Communities Grantee Convening

Please join us for lunch and a presentation on Health Forward’s Healthy Communities grantees and projects.

Healthy Communities 2017 grantees reported progress on their projects against our theory of change. We will share draft results from those reports as a group and look forward to discussing findings and interpretations of the results. You will also have the opportunity to share your experience with our theory of change, evaluation process, and reporting.

This event is for 2017 and 2018 Healthy Communities grantees. Up to three representatives from each organization who are involved in the program implementation, evaluation, and funding of Healthy Communities projects are welcome to attend. Grand Street Cafe provides off-street parking.
