Health Forward Awards More Than $870,000 in Applicant Defined Grants

KANSAS CITY, MO – The Health Forward Foundation (Health Forward) has awarded 16 nonprofit organizations a total of $872,954 in Applicant Defined Grants.

The following grants were awarded in February and March 2015:

Ad Hoc Group Against Crime $30,000 To provide the families of homicide victims with quality in-home mental health counseling services.
Amethyst Place $65,407 To support our supportive housing programs: education and technology, life skills and parenting, mentoring, and family support therapist.
Child Abuse Prevention Association $75,000 To provide the support staff needed to effectively facilitate CAPA’s Counseling program.
Child Protection Center, Inc. $63,537 To support the Child Protection Center’s Intake and Collaborative Family Support Expansion Program, which provides streamlined comprehensive intake, advocacy and supportive services for victims of child abuse and their families.
Coalition of Hispanic Women Against Cancer $40,000 To increase awareness and knowledge among Latinas about breast and cervical cancer prevention, early detection/screenings, and available community resources; and to provide follow-up to women to ensure proper continuum of care.
Developing Potential, Inc. $74,924 To support a full-time nurse to support adults with developmental disabilities who have complex medical needs.
Kansas City Free Eye Clinic $23,100 To support a full-time executive director position that will provide administrative and organizational services so more low-income, uninsured and homeless clients will have access to eye care.
Kansas City Metropolitan Crime Commission, Inc. $56,250 To support housing services for a vulnerable population in our community, returning offenders on parole in Jackson County.
KU Health Partners, Inc. $75,000 To support a collaborative program using a KC CARE Clinic community health worker to promote care transitions between KU Hospital Emergency Department and Silver City Health Center.
Medical Society Of Johnson & Wyandotte Counties Foundation, Inc. $40,000 To provide core operating support for this organization that increases access to specialty care services.
Midwest Foster Care & Adoption Association $65,000 To begin offering services which allow foster children with severe mental and behavioral problems the opportunity to transition to a forever home with a foster, adoptive or relative care provider.
Miles of Smiles, Inc. $45,000 To support the move of our permanent office location to a larger facility that will allow us to add a second operatory chair and complete restorative dental procedures more efficiently and with more patients.
Pets For Life, Inc. $7,500 To recruit, train, test, and certify additional volunteer/pet teams so we will be able to serve more individuals at additional facilities in our community.
SAVE Foundation, Inc. $75,000 To support linkages to care personnel for individuals on SAVE, Inc’s housing programs who are zero or low income and living with HIV/AIDS, mental illness and/or a substance abuse disorder. These staff members support clients in succeeding with housing and improving health care outcomes by linking clients to appropriate primary care or other medical services, as well as crucial supportive services such as job training, transportation assistance and nutrition support.
Secondary Trauma Resource Center $70,000 To build capacity in order to ensure sustainability and service delivery to Kansas City social service and health care agencies.
Swope Health Services $67,236 To support a bilingual Certified Application Counselor to serve Wyandotte County residents, especially Latino residents.

Health Forward Applicant Defined Grants (ADG) are awarded outside of the Foundation’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2015, Health Forward will award a total of $4.1 million in ADGs.
