Kansas City, MO — The Health Forward Foundation (Health Forward) has awarded eight nonprofit organizations a total of $398,417 in Applicant Defined Grants (ADG).
Health Forward program staff reviewed 20 applications, totaling more than $1.2 million.
The following grants were awarded in September 2016:
|Jacob’s Well Of Kansas City Ministries||$35,000||To support an occupational therapist position to provide services to severely disabled adults in a day treatment setting.|
|Kansas Children’s Service League||$35,000||To prevent and mitigate adverse childhood experiences with a focus in Wyandotte County through creating safe, stable, and nurturing relationships and environments.|
|Kansas City CARE Clinic||$40,076||To improve patient experience and communications.|
|Lafayette County Health Department||$75,000||To support the completion of the final three phases of the MAPP assessment process; results which will help reduce disparities and redundancies in service delivery, and connect people individually and collectively with resources and opportunities for health improvement. Health Forward funding will also support LCHD’s staff time to coordinate the Saving Smiles program and follow-up oral health care.|
|Lee’s Summit Cares||$62,467||To provide school-based therapy to uninsured and underserved students attending two Title 1 schools in the Lee’s Summit School District, and to provide intensive case management to families of children enrolled in therapy to provide academic support and access to social services.|
|Palestine Learning Center||$50,874||To support the development of peer-directed support groups and a monthly series of educational presentations to grandparents raising grandchildren residing in the urban core. Further, to support the empowerment of these grandparents in obtaining needed resources for their grandchildren and themselves.|
|Redemptorist Social Services Center||$60,000||To provide multiple types of medical assistance benefiting the medically fragile: the elderly, the underserved, the medically indigent.|
|Saint Luke’s Hospital Foundation||$40,000||To support Saint Luke’s Purple Fund, which provides critical outpatient rehabilitation therapies for patients are uninsured and unable to pay for such services.|
Health Forward Applicant Defined Grants (ADG) are awarded outside of the Foundation’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2016, Health Forward will award approximately $4.026 million in ADGs.
