Kansas City, MO — The Health Forward Foundation (Health Forward) has awarded nine nonprofit organizations a total of $367,057 in applicant defined grants during the month of June.
Health Forward reviewed 21 proposals totaling $1,219,942 for this funding round.
The following grants were awarded in June 2018:
|Vibrant Health||$48,057||To support implementation of the Community Health Improvement Plan (CHIP) Health Access project to improve equitable access to quality medical, mental health, and dental services for Wyandotte County residents.|
|Oral Health Kansas||$20,000||To improve the oral health of all Kansans through advocacy, public awareness, and education.|
|Comprehensive Mental Health Services||$40,000||To decrease behavioral health risk factors for Jackson County students through psycho-educational prevention groups.|
|Missouri Primary Care Association||$35,000||To support advocacy efforts that educate lawmakers and the general public about the role of federally qualified health centers in assuring access to health care for low-income and uninsured Missourians.|
|Duchesne Clinic||$49,000||To improve access to quality health services for vulnerable communities in Wyandotte County.|
|The Greater Kansas City Affiliate of Susan G. Komen||$35,000||To provide mammograms, screenings, and breast health education to uninsured and underinsured women in the greater Kansas City area.|
|Kansas City Community Gardens||$40,000||To empower families to plant community orchards by providing horticultural training for sustainable fruit production in neighborhoods with the lowest life expectancy.|
|Center for Developmentally Disabled||$35,000||To improve medical care and reduce the incidence of fall injuries for individuals with developmental disabilities by supporting a full-time registered nurse supervisor.|
|Seton Center Family & Health Services||$65,000||To provide dental services to those living at or below the federal poverty level.|
Applicant defined grants are reviewed and awarded monthly. These grants allow organizations to define their own immediate grant needs in relation to the Health Forward mission, target population, and service area. Organizations are allowed to apply once per year and are capped at $75,000. In 2018, Health Forward will award approximately $4 million in applicant defined grants.
