Health Forward awards $734,583 in Applicant Defined Grants

Kansas City, MO — The Health Forward Foundation (Health Forward) has awarded 15 nonprofit organizations a total of $734,583 in Applicant Defined Grants (ADG).

In July, Health Forward reviewed 21 ADG proposals totaling nearly $1.3 million in requests, and in August, Health Forward reviewed 17 proposals totaling nearly $1.1 million.

The following grants were awarded in July and August 2016:

The Bridge Home for Children $35,000 To support the capacity building and succession planning efforts of this urban-based youth behavioral health residential program.
City of Moran $25,815 To fund a public exercise center in Moran, Kansas, to increase access to fitness opportunities in a remote and underserved community in Allen County.
Compass Health $51,500 To provide needed behavioral health care services to underserved, underinsured and uninsured students of four rural school districts in Lafayette County, Missouri.
Health Partnership Clinic $75,000 To support a pediatrician at a new clinic location to provide pediatric services for the underserved.
Hope House, Inc. $39,267 To support the increased capacity of Hope House’s BridgeSPAN program which provides services 24 hours a day to patients identified as domestic violence survivors, provides training to medical personnel caring for victims, and provides assistance creating institutional policies and protocol to respond to the devastating consequences of domestic violence. Hope House currently offers the program at six area hospitals in eastern Jackson County.
Jackson County CASA $40,920 To enable CASA to help 150 older foster youth at risk of aging out of foster care achieve better health outcomes.
Jewish Vocational Service $75,000 To support the social work program at JVS that will provide support services to refugee and immigrant clients.
Kansas City Free Eye Clinic $31,300 To support administrative and organizational services so more low-income, uninsured, and homeless clients will have access to eye care.
Mattie Rhodes Center $61,175 To support the quality assurance of the Latino Mental Health program through accreditation, and clinical and licensure supervision.
Migrant Farmworkers Assistance Fund $50,871 To support the work of multiple staff members engaged in expansion of mental health case management in Lafayette County, Missouri, for Latino migrant farmworkers and their families, and for Latino migrant farmworker families now living there year round.
Healing House, Inc. $45,640 To support administrative staff positions and the purchase of new data management software and technology items to improve the organization’s data collection, tracking and reporting activities.
Thrive Allen County Inc. $62,000 To support development and financial staff: a deputy director and bookkeeper.
Unified School District 258-Humboldt $43,700 To continue the school health program and its policies, procedures, and activities that are designed to protect and promote the health and well-being of the school community.
The University of Kansas Center for Research $42,395 To support training, implementation, and evaluation of Strengths Model Case Management for youth with serious emotional and behavioral conditions at Johnson County Mental Health Center.
Uzazi Village $55,000 To provide partial salary support for the program director and executive director who will oversee and support perinatal community health workers (PCHWs) serving women enrolled in Medicaid and experiencing high risk pregnancies. Funds will also support technology for PCHWs in the field, and a new outreach worker who will be the program liaison in the community.

ADGs are awarded outside of Health Forward’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2016, Health Forward will award approximately $4.026 million in ADGs.
