Kansas City, MO — The Health Forward Foundation (Health Forward) has awarded six nonprofit organizations a total of $222,500 in Applicant Defined Grants (ADG).
For this round of ADG funding, Health Forward reviewed 17 ADG proposals totaling nearly $993,590 in requests.
The following grants were awarded in April 2016:

|Coterie, Inc.||$15,000||To deliver STD and HIV prevention and education presentations to adolescents in order to improve knowledge regarding HIV/STDs and reduce high-risk behaviors.|
|Cultivate KC, Inc.||$35,000||The Westport Commons Farm creates a place for neighborhood residents and businesses to connect with their food and with each other, while demonstrating the social, economic, and environmental value of urban agriculture by integrating food, art, job-training, and community space to breathe life into a repurposed public school in the heart of Midtown Kansas City.|
|Kansas Action for Children, Inc.||$75,000||To support a comprehensive advocacy strategy to shape health, education, and economic public policies on behalf of children and families in Kansas.|
|Kansas City Metropolitan Crime Commission Inc||$55,000||To provide case management and direct resource assistance for those participant parolees who are hard-to-employ and/or without viable home plans, to improve individual and community health outcomes.|
|Pets For Life, Inc.||$5,000||To recruit, train, test, and certify additional volunteer/pet teams to serve more individuals at facilities in the community.|
|Wyandot Inc.||$37,500||To equip 154 community-based case managers with iPads, which will allow concurrent and collaborative documentation, thereby enhancing service delivery, consumer and client interactions and efficiencies.|
Applicant Defined Grants are awarded outside of Health Forward’s annual Requests for Proposals. ADGs are reviewed and awarded monthly and allow organizations to define their own immediate grant needs in relation to the Health Forward mission. Organizations are allowed to apply for an ADG once per year and are capped at $75,000. In 2016, Health Forward will award approximately $4.026 million in ADGs.
