Application Process

Note: Health Forward has recently upgraded our technology and interface for applications and grants. All applicants, including returning applicants who had established accounts in our former grantee portal, must complete an eligibility quiz and register in the new system. This page contains all the necessary resources to start and submit an application.

The application process for foundation defined grants has seven steps:

  1. Health Forward issues a request for proposal setting forth the grant priorities.
  2. Health Forward releases a recorded webinar that details the priorities of the specific funding round, provides tips and resources, addresses frequently asked questions, and gives an overview of the application process. Watching the webinar is recommended for all applicants. An in-person conference hosted by Health Forward may also be available.
  3. Interested organizations submit a letter of intent.
  4. Interested organizations submit a full application.
  5. Health Forward staff and outside reviewers review the applications and conduct due diligence if necessary.
  6. Health Forward Board of Directors approves the grant awards for the funding cycle.
  7. Awardees will be notified of their funding by email, as well as by visiting the Health Forward website, or signing up for our press release email list.

Application Requirements

Deadlines
Letter of intent: Deadline has passed. Applicants who did not submit a letter of intent by noon on March 20, 2019 are not eligible to submit a full proposal.
Full proposal: April 17, 2019
Board approval: July 12, 2019

Letter of intent: May 22, 2019
Full proposal: June 19, 2019
Board approval: September 13, 2019

This zip file contains detailed instructions on how to apply using our new online system. You can use these documents to prepare all of your content for the Mental Health application.
