FDG Application Process

Application Process Note: Health Forward is working to improve and upgrade our technology and grantee interface for applications and reporting. We hope these improvements will deliver a more user-friendly and efficient process for all of our partners. Information about the new application process will be available in early 2019. The application process for foundation defined grants has seven steps:
  1. Health Forward issues a request for proposal setting forth the grant priorities.
  2. Potential applicants attend a pre-proposal conference hosted by Health Forward. The pre-proposal conference is an opportunity to ask questions, receive clarification, and network with other applicants.
  3. Interested organizations submit a letter of intent.
  4. Interested organizations submit an application.
  5. Health Forward staff and outside reviewers review the applications for completeness.
  6. The Health Forward Board of Directors approves the grant awards for the funding cycle.
  7. Awardees will be notified of their funding by letter, by visiting the Health Forward website, or signing up for our press release email list.

Download the Healthy Communities theory of change

  • Stage 1: Submit a Letter of Intent (Deadline is noon on March 20, 2019)
    • Begin an online Letter of Intent application
    • Download the Letter of Intent narrative template
    • Upload the following to your online application:
      • Completed Letter of Intent narrative template
      • IRS Letter of Determination (for 501(c)(3) orgs) or enabling statute/legislation or official description of purpose (for government agencies)

  • Stage 2: Submit a full proposal (Deadline is noon on April 17, 2019)
    • Log into your online account to access your full proposal application
    • Use the proposal narrative instructions to create your proposal narrative (updated narrative instructions are coming soon)
    • Download and complete the budget template
    • Upload the following to your online application:
      • 501(c)(3) applicants
        • IRS Letter of Determination
        • Copy of your organization’s most recent IRS 990 Report (nonprofit tax return)
        • Copy of your organization’s most recent financial audit
        • Board-approved current fiscal year operating budget for your organization
        • Roster of your organization’s board of directors with diversity information (race and gender)
      • Governmental entity applicants
        • Copy of enabling statute/legislation or official description of purpose
        • Copy of authorized budget or current financial statement
        • List of elected/appointed officials who oversee the entity’s performance
      • (Optional) Program logic model & outcome measurement framework examples
      • (Optional) Application checklist

  • Stage 1: Submit a Letter of Intent (Deadline is noon on May 22, 2019)
    • Begin an online Letter of Intent application
    • Download Letter of Intent narrative template
    • Upload the following to your online application:
      • Completed Letter of Intent narrative template

      • IRS Letter of Determination (for 501(c)(3) orgs) or enabling statute/legislation or official description of purpose (for government agencies)

      • Download the Outcomes Measurement Table and include the strategies, outcomes, and indicators (not included in page count)
    • Download and complete the budget template
    • Upload the following to your online application:
      • 501(c)(3) applicants
        • IRS Letter of Determination
        • Copy of your organization’s most recent IRS 990 Report (nonprofit tax return)
        • Copy of your organization’s most recent financial audit
