Applications are reviewed monthly by program officers (with the exception of December), with funding decisions made usually within 4-8 weeks after receipt of proposal.
No organization may submit more than one applicant defined grant proposal per calendar year, regardless of whether or not a grant is awarded. The maximum amount that can be requested is $75,000.
Awardees will be notified of their funding by email, visiting our website, or signing up for our press release email list.
Note: Health Forward has recently upgraded its technology and interface for applications and grants. All applicants, including returning applicants who had established accounts in our former grantee portal, must complete an eligibility quiz and register in the new system. On this page you will find all the resources you need to register and submit an application.
